Q: How do I become a patient?

A: Just click the “New Patient Request” button below. It only takes a few minutes, and we’ll handle the rest.*


*If you or your child needs to be seen right away, please call our office. Otherwise, thank you for your patience as we work through the many transfer requests we’ve received.

Q: What happens after I submit the New Patient Request form?

A: Once your request is received, our team will:

  1. Contact you to confirm details
  2. Help you request records from your previous provider
  3. Schedule your next appointment at Generations

Q: I’m already a patient at Rome Health Pediatrics. Do I still need to fill out paperwork?

A: Yes. Generations Family Health is a completely independent practice and not affiliated with Rome Health. We do not have access to your old records. Every family transferring here must either complete our New Patient Request form or call our office to officially establish care.

Q: Can you transfer my records for me?

A: We’ll walk you through the process, but only you (or your child’s legal guardian) can authorize the release of records. To make it easy, we provide a Record Release Form right here on our website. Simply complete and return it, and we’ll take it from there.

New Patient Paperwork

Q: Do I have to wait until my child’s next physical to transfer?

A: Not at all. You can transfer care at any time. If your child is due for a physical, we’ll schedule that visit. If they need a medication refill or follow-up, we can book an appointment for that instead. The sooner you transfer, the better — that way, if your child needs to be seen for a sick or urgent visit, we can get them right in without extra steps or delays.

Q: What insurances do you currently accept?

A: We are credentialed with the following insurance plans and can see patients under these coverages now:

  • Aetna
  • CDPHP
  • Fidelis Care
  • Medicare
  • Medicaid
  • MVP
  • Tricare (certified, in-network contract pending)

Q: Are there any insurances becoming active soon?

A: Yes!

  • Humana will be effective starting November 1, 2025.
  • Excellus BlueCross BlueShield is in the final stages of approval and should be active shortly.  

Q: Which insurances are still pending?

A: We are waiting on contracts with the following plans:
