Q: How do I become a patient?

Q: I’m a patient at Rome Health Pediatrics. Do I still need to fill out paperwork?
A: Yes. Generations Family Health is a completely independent practice and not affiliated with Rome Health. We do not have access to your old records. Every family transferring here must either complete our New Patient Request form, call our office, our book online to officially establish care.
Q: Can you transfer my records for me?
A: We’ll walk you through the process, but only you (or your child’s legal guardian) can authorize the release of records. To make it easy, we provide a Record Release Form right here on our website. Simply complete and return it, and we’ll take it from there.
Q: Do I have to wait until my child’s next physical to transfer?
A: Not at all. You can transfer care at any time. If your child is due for a physical, we’ll schedule that visit. If they need a medication refill or follow-up, we can book an appointment for that instead. The sooner you transfer, the better — that way, if your child needs to be seen for a sick or urgent visit, we can get them right in without extra steps or delays.

A: We are credentialed with the following insurance plans and can see patients under these coverages now:
