To save time at your first visit, please complete the following forms ahead of time.
Authorization for Release of Information:
This form gives us permission to request or share medical information when needed.
You can complete the form now and submit it along with your new patient packet.
Please note: We will not request your records from previous providers until our office officially opens.
Submitting the form ahead of time simply helps us prepare so that once we open, we can request records without delay.

Uploading Your Forms
To help us prepare for your visit and provide the best care, we ask that you submit your completed forms before your appointment by uploading securely, mailing, or dropping them off at the office. Bringing paperwork with you the day of your visit should be considered a last resort, as it limits our ability to review your information ahead of time.
Disclaimer: Our online upload tool uses a secure, encrypted system through Google Workspace (HIPAA-compliant) to protect your health information. Please use this option whenever possible. Standard email is not fully secure and may put your information at risk.

We accept a variety of insurances. We are still waiting for some insurance carriers to contract with us but hope to have them all active by October 2025!
